client portal/forms

If you are in need of admissions testing (child applying to an independent school, i.e. WPPSI, WISC, RIAS, WJ only) or IQ testing only, please contact our office directly. We are only seeing returning families. Do not proceed to the portal below.

For all types of diagnostic testing, Please click through to the client portal (below) if you are ready to get started. We have learned that it is well worth spending a little more time up front to truly understand your testing needs. You will first be asked to set up a login.  You then provide basic identifying and contact data that allows us to join you in planning stages. Only name, address, phone, email, and birth date are required before you can access the intake forms. Please keep in mind that this does not commit you to testing--it simply gives us the critical information we need to begin a meaningful conversation!

  • If you are an adult seeking diagnostic evaluation for yourself--or for another adult, please complete the adult intake through the portal below.

  • If you are here in need of diagnostic evaluation for a child or teen we would like to learn a bit more up front. Whether self-referred, or encouraged by school, pediatrician, therapist, etc., please complete the child & adolescent referral checklist through the portal below. You will have to scroll down through the Adult questionnaire to get to the right place.

  •  Please enter for testing services only. Do NOT enter the portal if you are seeking custody, medical readiness, or fitness evaluations, admissions testing for elementary independent schools, therapy only, or if you are in need of testing for children younger than 7 years. We may see some 6 year-olds for diagnostic work but request that you call the office first to discuss.

What happens next?   Your portal information will be received on our secured HIPAA-compliant data management system. We will review what you have shared and do our very best to get in touch with you within two business days--either by telephone or email. The preliminary information helps us to think about potential matches to a clinician, timing, and scope of evaluation before we reach out to learn more. If you're curious about the actual testing process, please see the common questions section or the admissions tab. Availability varies depending on the nature of the assessment needed. We appreciate your patience. The more we can understand up front, the smoother and more efficient the process can be.
