. If you are unsure of the best time to do testing, or which clinician might be the best fit, please feel free to skip to step 4.

- Click here to go to the portal and complete the admissions testing information sheet. Please note that you will have the option to print a copy for your records. If you do not wish to submit this information electronically, you are also welcome to print and mail or fax.
- Plan to arrive comfortably in time for your appointment (please pack and extra mask!) so that your son or daughter may have a few minutes to get settled in our waiting area prior to testing. Be sure that your child is wearing a well-fitted cloth mask (not a gaiter, bandana, or loose paper face covering). Keep all food and drinks with you in the waiting area so breaks can be taken safely.
- Testing appointments are conducted one-on-one with children. While we often invite families into the testing space to help get situated, caregivers are asked to remain in the waiting area or nearby downtown during test administration. Please note that within the building, there are multiple options. The first floor offers plentiful seating near the I street entrance as well as the Library Express and various eateries.
